Thank you for your interest in applying for a Florence Nightingale Scholarship.

Applications close 14th October 2018 at 17:00hrs.

IMPORTANT INFORMATION:

All applications are to be submitted electronically via this form. Please note you cannot save and continue, you must submit all requested information at once. We recommend to download and save both the application form and nominator form onto your computer.

Applicants are required to submit the following to complete their application:

• Upload completed application form – view form and save.
• Upload completed nominator form – view form and save.
• Upload an electronic photo (head and shoulders only and high resolution).
• Upload a copy of your NMC registration document.

Please rename your application and nominator forms prior to upload using your surname first name NMC number name of form i.e. Smith John 12345 Application form.

Please note the following:

• Applications that do not demonstrate commitment to the profession, improving patient and health outcomes, commitment to the Florence Nightingale Foundation Alumni will not be considered.
• Incomplete applications will not be accepted.
• Applications without line manager support will not be accepted.
